Licensing and Certifications
Be sure you have a clear copy of your license(s) and certifications. -
Drug Test and Background Check
MedWave will walk you through this process. -
Vaccinations
Immunizations generally required are PPD, MMR Titers, Varicella Titer, Hep B Titer, and Flu Vaccine. -
Physical Exam
Physicals are required annually and should be valid within one year of assignment start date. -
General Documentation
Valid driver’s license or government-issued photo ID, SS Card, etc.
